General Directions for the Reporting Form
  • Complete the form using the most current & detailed information available
  • If you do not have the patient’s social security number, please provide all of the other demographic information, including address & date of birth
  • If the patient has had a biopsy and will be having further surgery, we recommend you complete the form after the final procedure.
  • For an explanation of SEER Summary Staging please refer to the SEER Summary Section.
  • It is not necessary to submit forms for patients only seen in consultation or for second opinions. However it would assist MCR with casefinding if you would complete atleast the patient information and treating physician. That way MCR could follow-back to that physician for complete treatment information.
  • Mail forms at least quarterly to MCR at PO Box 718, Columbia, MO 65205 or fax forms to 573-884-9655. You may send them more frequently.

SEER Summary Staging – 2000:Summary staging is the most basic way of categorizing how far a cancer has spread from its point of origin. Summary staging has also been called General Staging, California Staging, and SEER Staging. The 2000 version of Summary Stage applies to every anatomic site, including the lymphomas and leukemias. Summary staging uses all information available in the medical record; in other words, it is a combination of the most precise clinical and pathological documentation of the extent of disease. This is the staging system required by the Center for Disease Control’s National Program of Cancer Registries (NPCR) as well as the North american association of Central Cancer Registries (NAACCR). Hospital registries approved by the Commission on Cancer use both the TNM Staging and the SEER Summary Staging.
